To register, send e-mail with the following information for each registrant:
- Full name
- E-mail address
- Full mailing address
Also, please answer two questions:
- Are you willing to share your name and e-mail address with the other conference attendees?
- Which would you prefer for lunch: Indian buffet or sandwich and salad?
Finally, let us know how you'd like to pay. If you need an invoice, provide the name, e-mail address and mailing address of the individual to whom we should send the invoice. If you would like to pay using PayPal or your credit card, please let us know to whom to send the payment request, including name and e-mail address.
